Dispensary system access request checklist

Access requests should answer four questions: who needs access, why, what scope is required, and when the access should end. A simple checklist makes those answers reviewable.
Record the person or role, system, location, requested permissions, business reason, start date, and expected end date. Do not share credentials to avoid the request process. The dispensary data access review provides a recurring review pattern. Keep the resulting procedure in SOP version control.
Grant the smallest role that supports the approved task. Separate viewing, editing, exporting, and administrative rights. If the request asks for broad access, require the owner to explain the specific work that needs it.
Name the business owner and technical approver. After provisioning, test only the approved functions and record the result. The request is not complete until the requester confirms access works without receiving extra permissions.
Use an end date or a review trigger for temporary access. Include role changes, leave, termination, and vendor completion in the removal queue. The CISA access control guidance offers practical identity and access considerations.
Keep the request, approval, test note, and removal evidence together. Sample open access monthly and investigate accounts with no current owner or business purpose.
